How do you order this sweet swag? Simple!

2. Print it out (it's two pages, but you only need to submit the back)

3. Fill out the back page

4. IMPORTANT: Make sure you've put your name, email, and phone number on the form, otherwise we can't arrange for the pick-up/delivery of your swag!

5. Make Checks payable to: South Shore PTO

6. Send in (or drop off) your form and payment:
   ATTN: PTO, South Shore School, 9135 School Rd, Port Wing, WI 54865

7. Sit back and relax while we work on your order! Once it's done, we'll arrange pick-up/delivery with you.
